REQUEST FOR PROPOSAL (RFP)

OPEN INVITATION FOR THE SUBMISSION OF PROPOSALS FOR THE UPGRADING, REFURBISHMENT, OPERATION, MAINTENANCE AND FINANCE OF PHALABORWA HEALTH CENTRE AS A PRIVATE HOSPITAL FACILITY THROUGH A PUBLIC PRIVATE PARTNERSHIP.

 

Request for proposal is hereby made to prospective service providers for submission as outlined

 

The Limpopo Department of Health and Social Development (LDHSD) has established the need to concession the operation, maintenance and upgrading of the Phalaborwa Health Centre to a private entity. .  The private sector will also be required to upgrade and refurbish the health centre. The Health Centre will then operate as a private health facility.

 

The purpose of the Request for Proposal (RFP) is to invite suitably qualified firms or consortia to submit proposals in accordance with the terms and conditions contained in the RFP documentation.

 

Proposals must be made in line with the prescripts of the Treasury Regulations as contained in Chapter 16 of the same regulations. It is further required of the prospective bidders to comply with the stipulated Black Economic Empowerment and Socio-Economic requirements outlined in the RFP documentation.

 

Copies of the RFP documentation are obtainable at the following offices:

 

Office D 3 at the Provincial Office of the Department of Health and Social Development.

The five (5) Provincial Advice Centres, namely; Vhembe, Sekhukhune, Waterberg, Capricorn and Mopani. Each document will cost R50.00.

 

Interested parties are required to attend a COMPULSORY briefing session and site inspection meeting in terms of the schedule outlined hereunder. Attending parties must ensure that they obtain an Attendance Certificate as proof of attendance.

Closing date:       28/09/2009 at 11h00 in the bid box situated at the Department of Health and Social Development Offices, 18 College Street, Polokwane.

 

The LDHSD reserves the right to reject all Responses or amend, modify, postpone, withdraw or terminate the Request for Proposals or at any time.

Utilization of Courier Services:

Bidders wanting to utilize courier services to obtain bid documents should take note that:

A reference number must first be obtained prior depositing money in the Departmental Bank Account.  The reference number is obtainable by calling one of the numbers:  (015) 293 6363 or 6269 or 6266.  The reference number must be quoted on the deposit slip.  The appropriate Company name, contact name(s) and numbers of the bidding company must be provided when collecting the document, bearing in mind that the relevant office opens at 09h00 to 12h30 and 14h00 to 15h30 respectively. Please note that the Department does not get involved in this process.

Next to Polokwane Provincial Hospital. Turn off left from GROBLER STREET (one way street adjoining the N1 road from Pretoria) into DORP STREET. Turn off right from THABO MBEKI STREET (one way street adjoining the R71 road from Tzaneen) into DORP STREET.  Go past Polokwane Provincial Hospital.  Turn off at first street to right, that is College Street.  Office number D3.
